Our team at HSHS Illinois Home Care promises to deliver compassionate, quality care to our patients and their families in our Hospice program. Decisions about your health can sometimes feel difficult or overwhelming. While you are in our care, we promise to live out our four core values, respect, care, confidence and joy. 

Our hospice program is a state-licensed, Medicare certified, and Joint Commission accredited program.

HSHS Illinois Home Care offers supportive, compassionate care that can bring peace, comfort and dignity to a patient in those last days of life. The goal of our hospice team is to make the patient as physically, emotionally, and spiritually comfortable, so they may remain at home as long as possible with minimal disruption to normal activities.

Hospice:

  • Provides support and care for those in the last phases of life-limiting illness.
  • Recognizes dying as part of the normal process of living.
  • Affirms life and neither hastens nor postpones death.
  • Focuses on quality of life for individuals and their family caregivers

 

Patients Who Qualify for Hospice Receive:

  • Physician Services
  • Nursing Care
  • Medical Equipment
  • Drugs for Symptom Control and Pain Relief
  • Short-term Hospital Care as needed
  • Home Health Aide Services
  • Social Services
  • Spiritual Counseling
  • Dietary Counseling
  • Grief Counseling for the family for 13 months after the date of death.
  • Volunteer Services

 

Everlasting Memories The Everlasting Memories Program allows us to grant wishes to patients enrolled in our hospice program and provide them with an event or day to cherish in their final moments, and for their families to remember once the patient has passed. We have granted wishes ranging from a meal out with family to a “last swim.” Ask your nurse or social worker for more information.
