Who We Are

Senior Services of Central Illinois (SSCI) is a not-for-profit organization supporting and serving seniors with non-medical services to promote independent living and enrich quality of life. Since 1967, Senior Services of Central Illinois has supported seniors living independently. Through offerings to support basic needs, encourage healthy living and enriching lives with socialization, the agency offers the convenience of meeting multiple needs in one agency. The work of Senior Services is important to the community and often crucial to the individuals it supports and serves. Care Coordination Unit

Sangamon, Logan, Mason & Menard counties

The Care Coordination Unit (CCU) provides a variety of services that are designed to assist individuals 60 and over remain independent and in their own homes for as long as possible. CCU will determine needs, set up appropriate services, and follow up to make sure services continue to meet needs.

 

Comprehensive Care Coordination Program

The Comprehensive Care Coordination Program consists of certified Care Coordinators who provide in-home assessments. Care Coordinators identify needs and strengths of each individual and develop client-directed care plans that promote independence and prevent premature institutionalization. During the assessment, eligibility for the Community Care Program (Homemaker, Adult Day Service, Emergency Home Response Service) and other programs is determined so individuals may make informed choices regarding their own care.

 

Choices for Care

Choices for Care provides choices for long term care services.  Choices for care caseworkers complete a needs-based assessment in a hospital setting for patients who need further assistance after discharge.  Upon completing the assessment, Choices for Care caseworkers review service options and costs, provide general information on nursing facility care as well as the Community Care program so individuals may make informed choices regarding their own care now and in the future.

 

Adult Protective Services Program (APS)

APS responds to vulnerable persons who are alleged victims of abuse, neglect, and exploitation. Case workers develop and implement a care plan in order to stabilize the situation and reduce the risk of further harm. The care plan may include existing legal, medical, and social service systems such as in-home care, legal assistance, emergency shelter, petitioning for an order of protection, law enforcement intervention, etc. for people 60 and over, or 18 and over with a disability.

 

Information & Assistance Program

The Information & Assistance Specialist directs individuals and/or family members to available services and community resources which meet the individual’s expressed needs.

 

Senior Health Assistance Program (SHAP)

The SHAP Coordinator is responsible for outreach, information & assistance, and enrollment activities related to the Pharmaceutical Assistance Programs such as Medicare Part D.

 

Volunteer Money Management Program

The Volunteer Money Management Program assists limited-income seniors with managing finances. Insured and trained volunteers are appointed to provide a variety of services such as assisting with opening/organizing mail, budgeting, checkbook balancing, bill paying, and/or managing monthly Social Security benefits.

 

Caregiver Specialist

The Caregiver Specialist program provides information and resources to caregivers of individuals 60 or over in Sangamon, Logan, Mason, and Menard counties.

 

Elder Assistance Services (EAS)

EAS is a program that provides one-on-one home or office visits for advocacy. EAS provides assistance in dealing with physical, family, or other related losses that occur as part of the aging process. Counselors provide assistance obtaining access to local, state, and federal programs such as Medicaid, Food Stamps, BEAM, and LIHEAP.

 

Daily Bread

Daily Bread is a midday meal program offered at 7 congregate sites in Menard and Sangamon counties. Daily Bread offers a nourishing meal, an opportunity for fellowship, informative programs and social activities. Home-delivered meals are available for individuals 60 or over who are home bound and on a general diet. Contact Daily Bread at (217) 528-4035 for eligibility determination (Income is not a factor). There is a voluntary donation of $5.00 per meal to help supplement partial grant funds. Meal sites include Athens, Auburn, Chatham, Greenview, Petersburg, Tallula, and the Springfield Senior Center. To reserve a congregate meal or arrange for home delivery, call (217) 528-4035 at least one day in advance.

 

Springfield Specialty Meals on Wheels (SSMOW)

SSMOW is a home-delivered meal program focused on doctor prescribed, specialty dietary needs (low sodium, diabetic, etc.). The cost of a lunch meal is $8.75.  Participants may add a breakfast meal to their delivery for an additional cost of $5.00. At this time, delivery is available in Springfield only. To sign up, call the Nutrition Programs at 217-528-4035, ext. 1124.

 

Senior Transport

Senior Transport provides transportation to participants, 60 years of age and over, as well as individuals with disabilities 18 years of age and over, living in Sangamon County. Vehicles are wheelchair accessible. Policies are provided to participants with information regarding priorities and daily scheduling of appointments.  Transportation is provided to rural county participants on a weekly basis and encompasses the entire Sangamon County area. Service for the greater Springfield area is offered Monday through Friday from 7:30 am to 4:00 pm. Reservations are required. There is a suggested donation of $2.50 per one way ride for individuals 60 or over, and a required fee of $2.50 per one way ride for individuals under the age of 60.

 

Foster Grandparent Program

The Foster Grandparent Program provides one-on-one mentoring, nurturing, and support to children with special or exceptional needs, or who are in circumstances that limit their academic, social, or emotional development. AmeriCorps Senior volunteers in FGP serve from 5 to 40 hours a week and receive hourly stipends. Volunteers must be age 55 or older and meet established income eligibility guidelines. In addition to the stipend, they receive accident, personal liability, and excess automobile liability insurance; assistance with the cost of transportation; recognition; and, as feasible, meals during their assignments.


Programs and Activities at “The Center”

Our Programs and Activities offer individuals a wide array of programs and activities such fellowship, dance classes, exercise classes, art classes, entertainment, crafts, health programs, volunteer opportunities, travel opportunities, and more!  The Gift Shop offers a variety of items handcrafted by seniors who come to The Center. SSCI invites you to come and participate in our exciting programs!


Trips & Travel Opportunities

Trips and Travel Opportunities offer individuals exciting and adventurous travel opportunities. Day trips are offered to nearby destinations such as St. Louis, Southern Illinois, Peoria, Chicago, and more. Extended trips are offered both nationally and internationally. Illinois Senior Games

The Illinois Senior Games promotes healthy lifestyles for individuals 50 or over. The games, which include more than 20 events and 3 team sports are an excellent way for older individuals to be fit, active, and improve their physical and mental health.  A great opportunity to socialize, enjoy some friendly competition, and engage the entire family. Springfield is the only qualifying site in Illinois for athletes that would like to participate in the National Senior Games. Visit the ISG website at www.illinoisseniorgames.org.
